How does it work?
Our benefits provide non-medical in-home care services through partnered home care agencies across the nation or through a friend or neighbor chosen by you. When you need care
- Call our toll-free number and speak with a Care Coordinator.
- Select Any Time Hours or Agency Hours and set your schedule.
- Receive the care you need within 72 business hours and we'll take care of the rest for you.
Any Time Hours
Service provided by a friend or a neighbor chosen by the member.
Service available
- Any day of the week
- Any hours including overnight
- 24/7 live-in home care
Direct deposit payment to the friend or neighbor caregiver is paid once a week on Fridays.
Agency Hours
Your care coordinator will help match you with a provider from a national network of Home Care agencies.

Step 1
Confirm you're eligible by answering YES to the following:
- Are you able to be left alone at home for a week or more and live independently at this time?
- Are you currently able to perform daily activities such as bathing, dressing, and transferring without assistance from anyone (including family members)?
- If you can answer yes to both questions, then you're ready to get started. Don't wait to purchase until the need is there because then you won't qualify.
Step 2
Decide on a plan that is right for you, then complete your enrollment online using our digital enrollment form. Please note that a new window will appear once clicked.
Enroll with a partner and receive an additional 10% discount on top of the discounted member rates below. Only the member is required to sign the enrollment.
This enrollment is a one-time opportunity, and while you have the flexibility to decrease your plan at any point, it is important to note that you will not be able to increase your plan once you enroll.
Bronze
The Bronze Plan includes 1,500 lifetime hours of care. It complements existing long-term care plans or suits those with limited care needs and budget.
Retail rate $95/mo or $1,140 annually
NYSUT discount rate $60/mo or $720 annually
Silver
The Silver Plan offers 3,000 lifetime hours of dedicated support, designed to accommodate a range of care scenarios with essential care.
Retail rate $175/mo or $2,100 annually
NYSUT discount rate $120/mo or $1,440 annually
Gold
The Gold Plan provides 6,000 lifetime hours of care. It’s tailored for those who desire generous support within a reasonable budget.

Platinum
The Platinum Plan offers unparalleled flexibility and the most comprehensive care, with 10,000 lifetime hours of care.

After your enrollment, you will receive the following:
- Your Field Issued Contract (enrollment application + a brochure) is your “policy”, which is emailed to you.
- A welcome letter with your ID card is mailed to you within 30 days after the enrollment.
